client treatment forms

Attached, you will find client treatment forms that will be completed and signed prior to the beginning of treatment. The attachment includes a professional disclosure statement (“Information for Clients brochure”), a Consent to Treatment form, a Client Information form, my HIPAA Notice of Privacy Practices, an Agreement for Payment form, and a Release of Information Authorization form. The first five are required to be read and completed/signed for treatment to begin.  The final form, the release of information, only needs to be completed if there is anyone who you would like me to be able to collaborate with, with regards to your/your  family’s  treatment (a previous  therapist,  a  family member who resides outside of your home, a doctor, a teacher, a school counselor, etc.). I am always happy to collaborate with other professionals or extended family members if the need exists / if it would be beneficial to do so. If you do not envision any collaboration needs at this time, we can always fill out a release of information if the time / situation arises. Please  let  me  know  if  you  have  any  questions  whatsoever  with  regards  to  these  forms.
